There is no way to set a default for turning on/off Header and/or Footer.
I'm sure a lot of people use this, I don't ever, leading to extra work turning off these.
A setting check-box for this could be included in the Tools > Options > OpenOffice Calc, or in the Format > Page > Header where selection takes place.
