Reproduce:
- document with two page styles active
  (First page for page one, Default for other page(s))
- choose Insert|Header|All (or Insert|Footer|All) from the menu

Result: no header (footer) appears.
Expected: header appears both on first and next pages.
Work around: Choose Insert|Header|First Page and Insert|Header|Default

Checked with 2.2.1 RC2 and RC1 (XP) and 2.2.0 (Linux)
I'm sure this has worked in an earlier version.

MRU->OS: as described, if there are more than one page style used in a document,
Insert.Header/Footer.All does not work.

The arguments in case of "All" is not a '?' but a '&'
The attached patch fixes this.
