When inserting a file name text field via API, the format of that file name
object is set correctly only for FULL and NAME_AND_EXT.  I'll attach a file with
macro to demonstrate the problem.

Please run the macro named putField.  It'll insert FileName field into left,
center and right header panes and the left footer pane, using different format.
