Right now, if I'm not mistaken, we can only create indexes for the entire
document and "Chapters" which are the first level of "outlining" in the document.

It's a big problem for my documents and I'm surprised that nobody else needs
more flexibility.

Let's say I have something like this:
Section A
    Chapter 1
      Heading 1
      Heading 2
      Heading 3
    Chapter 2
      Heading 1
      Heading 2
      Heading 3
    Chapter 3
      Heading 1
      Heading 2
      Heading 3
    Chapter 4
      Heading 1
      Heading 2
      Heading 3
Section B
    Chapter I
      Heading 1
      Heading 2
    Chapter II
      Heading 1

Now...At the beginning of each chapter, I'd like to have an index of all the
headings of the current chapter.

I couldn't find an easy way...Chosing "Chapter" will bring all the headings of
Section A.
The only way to do this was to put sections at the 3rd level of oulining, so
that chapters are at level 1 and are treated as the "Chapter" in the index
dialog. It's not logical.

My wish is to have a tool that lets us define the dropdown list in the index dialog.

I could then have this
Entire document
Sections
Chapter
Sub-chapter
anything else

Anybody could have any index they wish for.

Right!
When h having a tructure as:
"Section" -> Level 1
"Chapter" -> Level 2
"Heading" -> Level 3

Inserting a TOC after a "Chapter" with options "for Chapter" should only display
the "Heading" level of THIS "Chapter" and not all "Heading" of the document.

I regard this as a bug, not as an enhancement.

It absolutely works as designed. Creating an Index for a chapter means
collecting everything below a first level outline. That's what it should do. 

Creating indexes containing sub-chapters of a specified level would be an
enhancement.

The Issue you raised is currently assigned to 'Requirements' pending review, but
has not been updated within the last 3 years. Please consider re-testing with
one of the latest versions of OOo, as the problem(s) may have already been
addressed. Either use the recent stable version:
