Screenshots illustrating issue

On Standard toolbar there is an icon for table insertion.

Steps to reproduce:

1. View > Toolbars > Tools
   Insert toolbar appears.
   Notice icon with similar look as that one on Standard toolbar.

2. Click right arrow next to it.
   And now comes surprise!
   First icon is set looks exactly like two icons already mentioned.

3. To spice up this unusual phenomenon:
   - Click little arrow in the toolbar's title bar,
   - from menu choose "Customize toolbar"
    
   Customize dialog appears. Active is "Toolbars" tab page,
   toolbar "Tools".
  
   Just for better experience make sure that Tools toolbar 
   and Customize dialog do not overlap.

   Take a closer look at Commands list for Tools toolbar. You will spot
   that first command/item on list - "Insert" - has an icon
   and it is not what you see on screen!

5. Untick check box on the left side of this item.
   You'll notice that Tools toolbar has now one item less.

6. Tick checkbox
   Icon reappears on Tools toolbar but it is not that icon you see in 
   Customize dialog!


Really interesting case. I admit - up to now I haven't seen anything like that.
