reproduce steps:
1)create a presentation
2)create a new shape
3)right click of new shape, pop up context menu
4)Choose "Line"
5)Choose "Arrow Styles" Tab
6)Check "Add" button

Add button is disenable.

ALG: Cound not reproduce in current AOO build, the button does what it is intended to do, it uses the selected SdrObject as pattern for creating a new arrow head. It asks for a name and then adds the newly created arrow head. It may be a little bit hard to understand first, but the mechanism is quite flexible: You can draw whatever form You like, select it and create an arrowhead from it by using that button. It is also desribed that way in the help. Please recheck if it was a misinterpretation/wrong expected behaviour.
